Breaking News!
We are excited to announce that we have joined Axia Women's Health, one of the nation's largest independent women's healthcare groups, who shares a common vision to improve healthcare for women.
This opportunity will enable us to modernize our practice, expand our services, and maintain our physician-led independence. With the support of a national organization, we'll be better equipped to drive positive change in women's health and ensure that all women have access to the quality healthcare they deserve. By joining forces, we can continue to provide the outstanding, patient centered care you've come to expect from us, while reaching more patients in need.
Your physician and our location will remain the same. This new relationship will not affect any scheduled appointments or the phone number you call when you need care.
If you decide you do not want to continue your care with our practice, please visit axiawh.com/nepa-transfer to learn more about your options to have your medical records transferred to another physician. We look forward to announcing new programs and expanded service offerings to enhance your care in the coming months and sharing our updated website.
The majority of our accepted insurance plans will remain the same. One plan that is changing is UnitedHealthcare and as of November 1, 2024, we will no longer be an in-network provider. If you have an upcoming non-elective surgery scheduled, or follow-up care from a recent surgery, you may be eligible for continuity of care. Please contact our office or go to axiawh.com/uhc-nepa for more information. All continuity of care forms must be completed by November 30, 2024, and will be reviewed by UnitedHealthcare.
Finally, you will begin to see the Axia Women's Health name appear in our communications and branding going forward. Please remember that while you may notice an updated look, we are still the same team, devoted to providing quality, compassionate care for the women we serve.
We thank you for including us as part of your healthcare journey. We encourage you to also visit axiawh.com to learn more about Axia Women's Health and the network's impressive scope of services.
Warmly,
Lynne M Coslett-Charlton, MD, FACOG
